4. Cities in the Russian Federation you are supposed to visit.
5. Name, surname, and patronymic name (if applicable).
6. Sex.
7. Nationality.
8. Country and place (city) of birth.
9. Country and region (city) of residence.
10. Place (country and city) of Russian Federation consulate where the visa will be issued.
11. Current occupation.
12. Employer.
13. Employer’s address.
14. The copy of the valid passport (the passport should be valid 6 months after the date of arriving). The copy should be of good quality (fax copies are not valid).
